MS Athletics Eligibility
- Online registration is required for all new and returning athletes.
- Current TPS Health Physical Form filed either electronically on the Athletic Registration portal or with the school Health Office.
- Physical exams dated on or after April 1, 2023 are valid for the 2024/2025 school year.
- Post COVID19 Medical Clearance Form (For sports participation after positive test or symptoms of COVID).
- Parents and athletes are required to review the Concussion Fact Sheet. To comply with Wisconsin Concussion laws, parents and students review and electronically sign a Concussion Acknowledgement form during online registration.

US Athletics Eligibility
- Online registration is required for new and returning athletes and managers
- US Athletes require two current health forms: 1) WIAA Medical Eligibility Form and 2) TPS Health Form. Links to both appear in the forms section below.
- Physical exams completed on or after April 1, 2023 are valid for the 2024/2025 school year
- Please file all health related forms electronically on the AR portal and retain the original for your records.
- NOTE: Health forms will be rejected and the student will not be cleared to practice or compete if the exam date is omitted, expired, post-dated and/or if required signatures are missing.
- Each athlete and at least one parent are required to attend the pre-season meeting for their chosen sport(s) and season(s)
- Student Health Insurance The WIAA requires high school students who participate in high school sports to have health, injury or accident insurance. The Prairie School does not provide health insurance coverage for athletes. High School students without health insurance may not try out, practice or complete in any sport. If a high school student would like to participate in a sport and does not have insurance, families may request information regarding a school-year policy through K & K Insurance or any other insurance provider.

Each year, we see more and more of our student-athletes seeking information on opportunities to participate in college-athletics. We have dedicated this section to providing you resources that will allow you access to valuable information about the types of programs and the steps involved in seeking opportunities to play at the collegiate level. This page will continue to evolve and we hope to add sample resumes as well as alumni collegiate athletes’ comments on their college athletic experience. As always, we encourage families to contact their Head Coach. Working with the Prairie College Counseling Office and our coaches, we will assist you in this process. However, these opportunities cannot exist without the effort and energy of the family.

The National Student Clearinghouse, a non-profit organization, is the nation’s trusted source for student degree and enrollment verification. Any student pursuing college athletics, at any level, must register with the Clearinghouse. The Upper School College Admissions Office can help you complete this process. Through the Clearinghouse verification reporting solutions, they help educational institutions improve efficiency, reduce costs and workload, and enhance the quality-of-service they provide to their students and alumni, lending institutions, employers, and other organizations.
